Process

What To Expect

  • We will email you a link to our online portal.
  • Families input vital statistic information for the death certificate and cremation permit. 
  • We will email the authorizing agent(s) a cremation authorization form to be digitally signed using a phone/computer. 
  • Once the death certificate has been reviewed and approved, it is assigned to the physician for cause of death and digital signature.
  • We then apply for a cremation permit from the medical examiner in the county of death. 
  • Once we have the cremation authorization forms, death certificate, and cremation permit, we will schedule the cremation. The entire process usually takes a few days, but can take up to a couple of weeks depending upon how soon we are able to obtain the required documentation. 
  • Death certificates can be obtained from any Tennessee health department once the information has been approved by the informant and signed by the physician.
